Becoming A Patient
Thank you for your interest in our practice. We are currently accepting new patients. Please read through the frequently asked questions below. You can also search our help documents. If you still have questions, call us at (415) 334-1010, and we'll be more than happy to help you out.
1. Fill out the New-Patient Application Form
You can submit the form in two ways:
Electronic (Recommended) - Click here to fill out and sign the New-Patient Application Form digitally, and we'll receive a signed copy within minutes.
Download and Print - Click here to download the New-Patient Application form. You can email the form to firstname.lastname@example.org or mail it to Dr. Thomas S. Cowan, C/O Front Desk, 661 Chenery Street, San Francisco, CA 94131.
2. Call us to schedule your first appointment
After you've submitted the application form above, please call us at (415) 334-1010 to make an appointment. Note that a credit or debit card is required to make an appointment. We will not charge your card until after your appointment, but please be aware that we have a 24-hour cancellation policy.
Frequently Asked Questions
What are your rates?
The fee for an initial office visit is a flat rate of $375 ($350 for a phone consult, California only). Follow-ups are $40-$160 depending on time and complexity. We also have a Community Supported Healthcare Plan for those who are, or intend on being, long-standing patients.
Do you accept insurance?
We do not accept insurance at this point. However, we can give you a form to fill out and submit to your company for possible reimbursement. We also have a Community Supported Healthcare Plan of our own to help fulfill the need of affordable healthcare.
Do you accept new patients?
Yes! Dr. Cowan is currently receiving new patients.
How soon can I be seen by the doctor?
Dr. Cowan is usually booked 1-3 weeks out for new patient appointments. We regret if our schedule creates inconveniences. If your condition is urgent, we can fit you into the first opening after we receive a cancellation.
Should I come in or make a phone appointment?
Seeing patients in person has obvious advantages. However, if you live outside the Bay Area or are unable to travel, phone consultations are a practical option. Dr. Cowan treats patients nationally and internationally. (Note: Out-of-state initial consultations are currently unavailable. Dr. Cowan can do follow-up phone appointments after an in-person initial visit.)
